Understanding the difference: transcutaneous vs transconjunctival lower blepharoplasty

transconjunctival lower blepharoplasty

Lower blepharoplasty is a surgical procedure designed to improve the appearance of the lower eyelids, most often by reducing puffiness, smoothing under-eye bags, or refining the contour beneath the eyes. While the aim is similar in most cases, there is more than one way to perform the surgery. The two primary approaches are known as transconjunctival and transcutaneous lower blepharoplasty.

Although both techniques can produce excellent results in the right hands, they differ in how the surgeon accesses the tissues beneath the skin, the type of changes that can be made, and the way the eyelid heals.

Understanding these differences is essential for anyone considering surgery, as it helps set realistic expectations and ensures that the most suitable method is chosen for each individual’s needs.

What is transconjunctival lower blepharoplasty?

In a transconjunctival lower blepharoplasty, the incision is made on the inside of the lower eyelid, through the conjunctiva. This means there is no external scar. The approach allows the surgeon to access and adjust the fat pads beneath the eye without cutting through the skin or muscle at the front of the eyelid.

As it avoids an external incision, this method is often preferred for younger patients, or for those whose main concern is puffiness from excess or displaced fat rather than loose skin. It is also particularly useful in patients where skin quality is good and there is no significant sagging to address.

The recovery from transconjunctival blepharoplasty can be faster in many cases, with less risk of eyelid retraction or changes in eyelid position. Swelling and bruising still occur, but without a skin incision, there is no visible scar and the risk of altering the natural shape of the eye is lower when performed by an experienced surgeon.

What is transcutaneous lower blepharoplasty?

The transcutaneous approach involves making a small incision just beneath the lower lash line. Through this incision, the surgeon can access the underlying fat pads, muscle, and skin. This method not only allows for fat repositioning or removal, but also makes it possible to tighten loose skin or muscle at the same time.

For patients who have under-eye bags along with lax or crepey skin, the transcutaneous technique can address both issues in a single procedure. The incision is placed in a natural crease so that, once healed, the scar is typically very discreet.

As the skin and muscle are lifted during this approach, it can require a slightly longer recovery period than the transconjunctival method. There is also a slightly higher risk of temporary eyelid position changes during healing, which is why precise technique and careful aftercare are so important.

Choosing the right technique

The choice between transconjunctival and transcutaneous lower blepharoplasty depends on several factors, including skin quality, the amount and position of fat beneath the eye, muscle tone, and the overall anatomy of the eyelid.

If there is no significant excess skin and the primary concern is puffiness, a transconjunctival approach may be the most suitable. It allows for precise fat adjustment without disturbing the eyelid’s outer structures.

On the other hand, if there is loose skin, fine wrinkling, or muscle laxity in addition to under-eye bags, a transcutaneous approach may be more effective, as it enables both skin and muscle tightening along with fat repositioning.

An experienced oculoplastic surgeon will assess the eyelids carefully, sometimes using photographs or imaging, to determine the most appropriate method. It is not unusual for different patients with similar concerns to require different techniques based on the fine details of their anatomy.

Why specialist expertise matters

Lower eyelid surgery requires a detailed understanding of the delicate anatomy around the eye. The skin is thin, the muscles are finely balanced, and the structures beneath are closely connected to the eye itself. Choosing the wrong technique, or carrying out the right technique without the necessary precision, can lead to problems such as eyelid retraction, asymmetry, or changes in eye shape.

A surgeon with specific expertise in eyelid procedures can recommend the safest and most effective approach, taking into account not only the desired cosmetic outcome but also the long-term health and function of the eyelid. Careful planning, meticulous technique, and close follow-up all contribute to achieving a natural, harmonious result.

Both transconjunctival and transcutaneous lower blepharoplasty are established techniques for rejuvenating the under-eye area. While the transconjunctival method offers a scarless approach ideal for younger patients or those without skin laxity, the transcutaneous method allows for more comprehensive correction when both fat and skin need to be addressed.

Selecting the right approach is not about choosing the ‘better’ operation, but about matching the technique to the individual’s anatomy and goals.

With careful assessment and the skill of a surgeon experienced in both methods like Dr Nick Koutroumanos, lower blepharoplasty can restore a smoother, fresher appearance while maintaining the natural character of the eyes.

I am now two months post–upper and lower blepharoplasty and brow lift, and I can honestly say that I am beyond thrilled with my results. As this was a significant procedure, I spent a great deal of time researching and consulting with several surgeons before making my decision. However, from the moment I met Dr Nick, I felt completely safe, confident, and reassured. His professionalism, expertise, and calm manner immediately put me at ease, and I had no hesitation in booking my surgery with him. The entire experience has exceeded my expectations. Dr Nick is an exceptionally talented and meticulous surgeon, and the results speak for themselves. Equally impressive has been the level of aftercare. Throughout my recovery, he has been available, thoughtful, and responsive to any questions or concerns I had, providing support and reassurance every step of the way. I could not be happier with my outcome and would recommend Dr Nick without hesitation to anyone considering facial surgery.

I went to Dr Koutroumanos to discuss a third ptosis surgery, and I’m so glad I did. Nearly three months on, I am thrilled with the results of my surgery. Nick was extremely thorough and meticulous, especially in comparison to my previous repairs (which were not performed by him). Dr Koutroumanos is very approachable, and I wouldn’t hesitate to recommend him to anyone considering surgery, particularly those with ptosis who have had unsuccessful procedures in the past. I had almost resigned myself to living with my ptosis, but Dr Koutroumanos has changed that and everyday I look in the mirror and feel so much more confident.
